Six of the eight questions feed four areas. Each answer scores from 0 (no leak) to 3 (heavy leak); the four areas are weighted equally and the average becomes your score out of 100. The first two questions only pick your template and the number of requests used in the cost estimate.
This is a self-assessment, not an audit. Everything is calculated by a small script in this page; there is no form submission, no tracking and no storage.
